Request for one-time building access during COVID-19 closure

Access to academic buildings on campus is restricted during the coronavirus pandemic for personel safety and to reduce the spread of the virus.    

All personnel coming to campus must complete the online COVID-19 Symptom Check for Employees and have been issued a certificate of clearance to work on-site. 

***Effective Fall Quarter 2020, UC system policy requires that all students (on-campus, off-campus and international), faculty, and staff working on campus are required to receive a flu shot by November 1, 2020 and must complete the Influenza Vaccination Attestation Form by October 31, 2020.*** More information about the policy can be found on the Student Health Center website and in the campus-wide announcement from August 7, 2020.

For one-time access to the Jack Baskin Engineering Building, E2, and Westside Research Park, please complete the request form below. One-time access requests to these three buildings are reviewed by Baskin Engineering Facilities Manager, Tony Sabala. Whenever possible, requests should be submitted with 2 full business days to allow time for review.

For access to the Physical Sciences Building, the Biomedical Sciences Building, and Sinheimer Labs, email your request to PBSci Assistant Dean Sarah Barron ( The email should identify the person requesting access, the business need for entering the building, the rooms the person will enter/work in, and the dates and times the person plans to be in the building.

Faculty should list their department chair. Staff should list their supervisor. Researchers should list their PI or faculty adviser.
Please provide the reason why you need to enter the building. Generally requests to do work in the building that could be done remotely (e.g. computer work) are not considered to be appropriate during the COVID-19 closure unless there are sufficient extenuating circumstances. This request will be evaluated based upon this rational by Baskin School of Engineering Dean's Office. Please allow two business days to evaluate your request.
